1. Open client profile
2. Click the Assessments from the left sidebar
3. Click Assessment History
4. Click the Services toggle in the main screen
5. Locate the scale to which you would like to add a service
6. Search services provided by using the Direct, Referral, or All tabs
7. Select the service from the drop-down list
8. Click Add
9. Service is now assigned
