Know Your Form Choices!
Follow this handy guide to help you fill out choices (answers) to your custom made e-form!
Radio: enter one answer per choice - yes and no answers will have two Choices:
- First choice = “Yes”
- Second choice = “No”
Checkbox: enter one answer per choice - enter one Choice (answer) for each:
- Diabetes
- Heart disease
- History of blood clots
- Asthma
- etc.
Textbox: enter one answer per choice - enter one Choice (answer) for each:
- Emergency Contact Name
- Relation
- Phone Number
- Address
- etc.
Drop-down: enter one answer per choice - enter all applicable choices, one answer for each:
- Less than 9th Grade
- 9th - 12th Grade (non-grad)
- High School Diploma or Equivalency
- Some College
- Associate Degree
- Bachelors Degree
- Masters Degree or PhD
Large Textbox: enter one answer per choice - some Large Textbox responses do not need a Choice entered. If the Field Name were, “Please explain the situation” you would not need to Add Choice. If the Field Name were, “Tell us about yourself:” Three Add Choices require long answers and could be:
- “I gain enjoyment from...”
- “I do not gain enjoyment from…”
- “Growing up, I wish …”
