This feature will not be available to add to other forms. If you wish to create a custom field to be used on other forms, see Custom Fields

1. Click Agency Admin from the System Navigation Bar

2. Click on the Forms tab

3. Locate the form you wish to edit

4. Click Edit inside the form’s box

5. Click Add Row in the main window

6. Click Create New Field

7. Click the pencil icon

8. Select your Field Type

9. Enter the Field Name 

The Field Name is the question - Example: “How many people live in your household?”

10. Click Add Choice 

Choices are answers to your question. For help on adding Choices, click here to Know Your Choices!

11. Click Save

Repeat steps 5-11 for each new custom field
