Important Note: You will only be able to edit and maintain Assessments that are owned by your agency. By toggling Source: Inherit to Source: Own you are turning off all Inherited items and they will not be available in client profiles. Click here to learn more.


1. Click Agency Admin from the System Navigation Bar

2. Click the Assessments tab

3. Select the Assessment Scales tab on the left

4. Locate the scale you wish to edit

5. Click the gear icon

6. Scale Type will automatically be set as Individual - click the drop-down menu and select Family if this is a Family scale - click here for more on Scale Type: Individual -vs- Family

7. Under the Scale Options section, Type Option Statement... under ADD NEW OPTION

8. Enter Option Value… tied to that Scale Statement -  any "N/A" scale statements MUST have a value of negative one (-1)

9. Click Add Option

10. Option Statement will appear in list on right under the EDIT/SORT OPTIONS toggle

11. Repeat steps 7-10 for all Scale Statements

12. After all Option Statements are added, you can rearrange the order they appear on the Assessment by clicking and dragging the Option Statements up or down the list under EDIT/SORT OPTIONS

13. System will save automatically
